Why "Care Coordinator"? As a licensed Speech-Language Pathologist (SLP) and Care Coordinator, with your consent, we are able access medical reports & collaborate with healthcare professionals to provide seamless, informed care. Services are HIPAA-compliant & specialized in areas of cognitive/memory and neurological conditions .
What It Allows:
- Access Medical Info – Review diagnoses, meds, & history from doctors/facilities
- Weekly Support Role – Coordinate activities, advocacy, & check-ins (remote or in-person)
- Advocacy Power – Speak for patients in Home/Facility settings
